Some changes have been made to the ‘Consent for Evaluation’ portion of the database. These were made largely to accommodate the new workflow for the FBA/BIP process, and I encourage you all to review them.
Reason for Evaluation:
An “Other” field has been added under the dropdown list for “Reason for Evaluation” This can be used in the event that additional assessment is required as part of anything other than initial eligibility or continued services, such as an FBA or BIP.
Evaluation Type:
While an “Other” option has been added to the reason for evaluation, it has been removed from the “Evaluation Type” field and replaced with “Additional Assessment”. Choose this option when obtaining consent for additional assessment of the student.
Terminology:
The term “Intellectual Ability” has been replaced with “Cognitive Processing”
