New Secondary Transition Checklist

In response to new state reporting requirements, we have added a “Transition Checklist” to the Secondary Transition section of the IEP database.   At the bottom of the transition screen, you will now see 8 questions that must be answered for every transition-age student.  These questions are aimed at ensuring that every student’s plan is complete, current, and appropriate.

At this point, these questions will be blank for every student in the database.  Because there is an error check associated with these fields, all IEP paperwork for transition students will print in DRAFT mode until these questions are completed.  If you need to print IEP paperwork for transition-age students, please take a few minutes to complete this transition checklist first.

The data in each of these fields will be cleared annually (just prior to the annual IEP) to ensure that these questions are reviewed each year and student transition plans are complete and up-to-date at each annual review.
